Nursing – Our registered and licensed vocational nurses coordinate care services to monitor patients' progress,
including pain management and health teachings. Home Health Aides –
Our certified home health aides assist patients with bathing and activities of daily living.Physical Therapy – Our physical therapists manage and provide instructional programs to increase patients'
ambulation and mobility. Occupational Therapy – Our occupational therapists
evaluate and provide ways to increase muscle strength to aid patients with their activities of daily living, allowing them
to be as independent as possible. Speech and Language Pathologist –
Our licensed speech pathologists assess a patient’s ability to communicate and provide instructions to regain communication
skills. Techniques of swallowing will also be provided. Medical Social Work
– Our certified medical social worker make the necessary coordination to make use of the available community resources.

all of our patients. Each patient is assigned an advocate that ensures the patient understands their services and
that our staff is always providing the highest quality care. They will conduct surveys after admission and during their
time of service with our agency. In turn, we use this information to continue to improve our quality of care and make our
patients experience with home health a positive one. |

Attention Discharge Planners - All you need to make is ONE phone call to us! We will evaluate your patients needs and if home
healthcare is not suitable for them, we will coordinate with our partners for hospice care, nursing home placement or rehab,
assisted living facility or other senior living arrangements. You can be assured that your patient will receive the highest quality care and will
be followed by us until the optimal outcome has been achieved.
